Appointment requests collect only the contact and scheduling information you choose to provide so the request can be emailed to Enhanced Health Solutions.
Please do not submit sensitive medical information through the scheduling form.
The website does not retain appointment requests in a customer database. After a request is validated and successfully emailed, the website does not intentionally keep the submitted name, phone number, email address, requested date, or scheduling note.
Email credentials and public practice configuration are stored server-side and are not exposed in browser JavaScript. Public production hosting should use HTTPS and appropriate server security controls.
